ConnectIE soared in 2023! Inland SoCal United Way 211+ enrolled thousands of new community members and IEHIO welcomed new healthcare organizations into the Community Information Network. Desert Healthcare District represented ConnectIE all throughout the Low Desert and IEHP began outreach to community based organizations and attending community events through the IE. Together, we improved support and made technological strides - including integrating into CareDirector and launching self enrollments for the Community Infomation Network!

ConnectIE has completed its integration of CareDirector, the care management platform used by IEHP‘s Health Homes Program! This integration will enhance coordination, streamline workflows, and improve data sharing for all healthcare organizations using CareDirector as their care management software.
We look forward to the positive impact a smoother care management system with community referrals will have on patient outcomes.

It's a friendly and efficient way to enhance their whole person care journey. Let's work together in making health more accessible and connected for everyone. Community-based organizations (CBOs) are pivotal in ConnectIE's mission to enhance whole-person care in the Inland Empire, Mountain, and Desert Communities. Providing essential social services, CBOs broaden access to crucial resources. Within the ConnectIE platform, CBOs can navigate to ensure timely closure of incoming referrals, assisting NGOs and local government in evaluating public needs and resource availability.
Inland SoCal 211, in example, is instrumental to the local community, serving as a comprehensive resource hub, offering social services, navigational assistance, and facilitating CIN enrollment. They use ConnectIE to keep track of the people they help, as well as the status of each community member’s referral. This helps 211 better assist the community by being well informed with contextual information and it helps keep track of resource matching, so that community members are connected to programs or services that can serve them best. 211's flexible, community-first role exemplifies how CBOs bridge needs gaps and how they can assist individuals in finding the right resource for them.
Closing the loop is vital for CBOs on ConnectIE, ensuring efficient resource utilization. Reach out to IEHP's Community Health Representatives at ConnectIERequest@IEHP.org (or reach out to us via ConnectIE’s Contact Us page!) to streamline your incoming referrals, set up program management, and master the platform for public engagement. This direct connection facilitates coordination, enhancing your impact on the community.
